It is a critical assessment for individuals who struggle with feeling tired or falling asleep unexpectedly during waking hours. This full-day procedure typically follows an overnight sleep study and helps doctors diagnose specific neurological sleep disorders.
The MSLT measures how quickly a person falls asleep in a quiet environment during the day. It is used to identify disorders characterized by abnormal or intense levels of sleepiness.
While some sleep tests focus on diagnosing breathing problems, the MSLT concentrates on measuring the patient’s biological drive to sleep.
The MSLT is a full-day test consisting of five short "nap trials". These trials are scheduled approximately two hours apart throughout the day. This procedure is generally conducted in a supervised setting, such as a sleep clinic or laboratory.
The patient is asked to lie quietly in a bed during each trial and try to fall asleep. Sensors are placed on the head, face, and chin. These sensors monitor when the patient is asleep or awake, and critically, monitor brain and muscle activity to determine if they enter dream (REM) sleep.
The goal of each trial is to measure the amount of time it takes for the patient to drift off after the lights are turned off. If the patient falls asleep, they are awakened after 15 minutes. If they remain awake, the trial is ended after 20 minutes. The first nap trial usually begins about two hours after the patient's mandatory overnight sleep study ends.
The ideal candidates for an MSLT are those struggling with excessive daytime sleepiness. These are individuals who feel exhausted at times when others are typically alert—such as when driving or working—without any clear, apparent reason.
The MSLT is deemed essential for patients suspected of having narcolepsy or idiopathic hypersomnia (disorders causing severe, unexplained sleepiness).
The data collected across the five trials helps the sleep physician diagnose hypersomnolence disorders. Specifically, the physician uses the measured speed of falling asleep and the timing of dream (REM) sleep entry to diagnose conditions like narcolepsy or idiopathic hypersomnia.
The results of the MSLT provide the foundation for a formal diagnosis and guide the doctor in determining the appropriate treatment plan for managing persistent, excessive sleepiness.
